Forms to Download

Required Forms for All New & Returning Students (if you completed these for summer camp, you are covered!):

• Child’s Application for Childcare (“Hospital Preference” must be completed with the name of a hospital)
• Permissions & Acknowledgements Form (Within this form you acknowledge you have received a Summary of the NC Child Care Law & Rules)
• Discipline and Behavior Management Policy
• Child’s Immunization History* (Immunization records do not have to use this form and can be faxed to 919-848-1571)
• Child’s Medical Report* (This has a portion for parents to complete and sign, and a portion for doctors to complete and sign)
• Nutrition Opt-Out Form
• Child Information Sheet

* Medical report and immunization records do not need to be completed for returning children at the beginning of the school year, but they must be updated with each well visit, each time the child receives an immunization, and whenever the child’s medical condition changes, such as a diagnosis of a chronic condition.

• Diabetes Action Plan
• Food Allergy & Anaphylaxis Action Plan
• Seizure Action Plan
• Infant Feeding Plan (required if child is less than 15 months old)
• Permission to Administer Medication for Chronic Medical Conditions and Allergic Reactions (Must complete all sections of the form with specific instructions. We cannot accept forms which say “as needed,” for example.)
• Permission to Administer Topical, Over-the-Counter Medications (Must complete all sections of the form with specific instructions. We cannot accept forms which say “as needed,” for example.)
• Automatic Bank Draft Form (required for month-to-month payment option)
